تحلیل نظام‌مند پژوهش‌های حوزه خط‌مشی‌گذاری سلامت در کشورهای در حال توسعه: رهنمودهایی برای پژوهشگران

چکیده

علی‌رغم توسعه مبانی نظری تحلیل از سیاست‌گذاری عمومی در حوزه سلامت کشورهای توسعه­یافته و استفاده فراگیر از آن، کاربست این مبانی در کشورهای در حال توسعه در مرحله ابتدایی به سر می­برد. در این مقاله بر مبنای مرور روایتی مقالات منتشرشده در حوزه تحلیل از سیاست‌گذاری سلامت در این کشورها بین سال­های 1994 تا 2017، گستره موضوعی، رویکرد روش­شناسانه و رویکرد تحلیلی مورد استفاده پژوهشگران بررسی و تحلیل شده­اند. هدف از تدوین این مقاله آن است که از طریق نشان دادن نقاط ضعف روش­شناختی و رویکرد تحلیلی مقالات و تشویق پژوهشگران حوزه سیاست‌گذاری سلامت کشور به انجام پژوهش­های بیشتر در این زمینه، به توسعه این حوزه نظری کمک کند. برای این کار مقالات بر اساس ۵ اولویتِ جستجو شناسایی و موارد منطبق با معیارهای ورود، شامل مجموعاً ۱۵ پژوهش انتخاب، مطالعه و تحلیل شدند. مقالات به لحاظ تعداد کم و به لحاظ گستره موضوعی متنوع بودند. اکثر مطالعات فارغ از بررسی چرایی تغییر سیاست‌ها به توصیف روایت­وار رویدادها، ایجاد نظم مفهومی میان عوامل تأثیرگذار و یا آزمون مجموعه‌ای از فرضیات بسنده کرده بودند. به لحاظ روش­شناسی نیز عمده پژوهش‌ها در قالب مطالعه موردی و بر روی یک سیاست خاص انجام شده و طرح پژوهش دارای نواقص زیادی بود. به لحاظ رویکرد تحلیلی نیز اکثر مطالعات از نوع توصیفی- تحلیلی بوده و به جای تبیین فرایند عمدتاً بر توصیف رویدادهای تاریخی و آزمون فرضیات تمرکز کرده بودند. این در حالی است که درک عمیق موقعیت­ها و چگونگی شکل‌گیری سیاست­ها و پیامدهای حاصل از آن به ‌واسطه تبیین پدیده تغییر سیاست می‌تواند به تقویت تدوین و اجرای سیاست‌ها کمک کند. بررسی این مطالعات نشان داد که تلاش­های صورت‌گرفته تاکنون به لحاظ روش­شناسی و رویکرد تحلیلی برای تبیین تغییرات سیاست در حوزه سلامت این کشورها بسیار محدود بوده است؛ به گونه­ای که درک پدیده سیاست‌گذاری در این کشورها به طرح­ریزی پژوهش­های عمیق کیفی برای توسعه نظریه­های موجود یا ارائه نظریه­های جدید یا قدرت تبیین­کنندگی بالا نیاز دارد. نتایج به‌دست‌آمده از این پژوهش پژوهشگران را به مطالعه سیاست‌گذاری به عنوان یک پدیده بسیط با بهره‌گیری حداکثری از رویکردهای پژوهش استقرایی برای حرکت به سمت توسعه مدل­های نظری جدیدتر با قدرت تبیین­کنندگی بالاتر یا تلاش برای بومی­سازی نظریه­های موجود برای تبیین دقیق­تر تغییرات سیاست سلامت در کشور رهنمون می­سازد.

Systematic Analysis of Research Articles on Health Policy in the Developing Countries: Implications for Researchers

چکیده [English]

In spite of development of public policy analysis in health domain in developed countries and its widespread use, its application is still in its infancy in the developing countries. In this article, based on the narrative review of articles published from 1994 to 2017 on health policy analysis in these countries, their thematic scope, methodological and analytical approach are analytically reviewed. This article provides a platform to assist researchers to develop this field by examining the methodological and analytical pitfalls, as well this study aimed at encouraging the health policy researchers to focus more on this research area. All relevant articles were identified according to five predefined search priorities. 15 articles fall into the review inclusion criteria and then they were studied and analyzed. The selected articles were small in number but varied in the scope. The majority of studies provided description of some episode of policy change. They explained a conceptual ordering among lots of influencing factors or tested a set of predefined hypothesis, regardless of exploring the reason behind health policy changes (policy process). In addition, most of the studies adopted a single case study design and had significant analytical weaknesses. As well, from the points of analytical reviewing, most of them were descriptive- analytical instead of being exploratory in nature. However, understanding factors influencing health policy process and the results of policy change deeply can strengthen future policy development and implementation. This study showed that the efforts made in developing countries to explain changes in health policy process have been limited. So, future qualitative studies with a rigorous study designs are needed to develop current theories or coming up with new ones with the aim of providing a better understanding of health policy process in developing countries. Health policy researchers are encouraged to study the health policy process as a whole, using inductive research approaches, to develop new theories rich in exploratory power or to localize current theories in consistence with national contextual requirement.
